Liver and kidney functions, electrolytes normal: LDH 318 U/L: Chest and abdominal CT show "gallstones" and "mild splenomegaly."
According to Western medical examinations, apart from gallstones, Pan Yuqian's body is very healthy and has no problems at all, yet she mysteriously develops a fever.
This was the first time Lu Xuan had encountered such a situation, and he couldn't help but furrow his brow slightly, saying to Ji Xiuwen, "Take her temperature."
Listening to Pan Yuqian's account and looking at the dense lab results, Ji Xiuwen's scalp was tingling. Hearing Lu Xuan's words, he snapped back to reality and casually grabbed the ear thermometer next to him to take Pan Yuqian's temperature, "Temperature 39.3°C."
Pan Yuqian responded, "Dr. Lu, it's just like this. It's a little better in the morning, generally staying below thirty-eight, but after noon, the temperature quickly rises, rarely below thirty-nine. I feel cold when the fever rises, and only sweat a bit when it recedes. I feel particularly thirsty..."
Pan Yuqian continued to describe some of her symptoms, and Lu Xuan seized the opportunity to observe her condition and recorded the diagnostic process in the patient record:
July 18, initial consultation temperature 39.3°C, pronounced in the afternoon, slight chills when heating, sweat when cooling, body temperature gradually declines at dusk, dry mouth with desire to drink, tongue dark red, thin greasy yellow coating, pulse...
Seeing Lu Xuan recording some of her conditions, Pan Yuqian asked, "Dr. Lu, what on earth is going on with me? The test results are all fine, but I just keep having a fever, and no antipyretic works. I've been resting for over a month without going to work, and it's really affecting my life."
After speaking, Pan Yuqian looked quite troubled.
Anyone in this situation would likely feel unsettled.
A fever can be shrugged off; after all, most ordinary people fall sick once or twice a year, and fever is quite common.
But it's been over a month with this fever, and all kinds of medicine have been tried; hospitalization didn't help either. A comprehensive check-up was conducted specifically for this, and apart from mild gallstones, there was no other issue.
When she got the results, Pan Yuqian felt frustrated for a long time.
She even wondered, if her body was fine, then why would she develop a fever for no apparent reason?
She presented the problem to Lu Xuan, but even he couldn't offer a good explanation at the time. Except for the ease of judging the pulse rate, he had never encountered such a strange pulse condition like Pan Yuqian's. Left with no choice, he rarely sought help from his so-called omnipotent mentors.
And this group of mentors had never let him down.
[The soggy pulse is categorized as a complex pulse condition, integrating floatation, fine texture, and weakness, hence the ancient saying "floating foam on water." This definition became clear only in the "Pulse Scripture."
Before the "Pulse Scripture," the soggy pulse merely implied weakness, as the "Inner Canon" mentioned Spring pulses being gentle and inviting, long-summer pulses being gentle, etc. In Zhongjing's "Pulse Diagnosis Method," it is stated: "Yang pulses are floating, large and soggy, Yin pulses are floating, large and soggy." The Eastern Liver Meridian is "thin, wiry, and soggy weak."
And so forth, showing that the soggy pulse and a weak pulse are very close in meaning, both indicating softness, weakness, and smallness.
In ancient texts, the soggy pulse is often described as soft; however, modern usage prefers the term soggy pulse.
The "Pulse Scripture" says: "The soft pulse is exceedingly soft and floats finely."
Below, there is a small footnote saying: "Some days, upon pressing, there is nothing, lifting reveals excess. Some days fine, soft, with soft written as soggy, described as soggy like silk in water, easily perceptible with light touch."
The "Pulse Scripture" thus established the standard for the soggy pulse.
In "Pulse Formula Errors," the soggy pulse is documented as "extremely soft and finely floated, discerned with a light touch, pressing should not be attempted."
Li Shizhen's "Benhu Pulse Study" also mentioned: "The soggy pulse is extremely soft and finely floated, like silk in water, discerned with a light touch, nothing is present when pressing, like floating foam on water."
"Medical Essentials" states: "The soggy pulse is floating, fine, and weak, appearing with light touch, disappearing with heavy touch, like a thread floating in water."
...]
Upon hearing this, Lu Xuan took a deep breath, keeping all these in mind while summarizing what Heart's Voice said.
According to Heart's Voice, the soggy pulse is a composite pulse condition; it integrates floating, finely textured, and weak characteristics; due to its floating, finely textured, and weak nature, when felt, it is light, soft, and small, thus described as a "thread floating in water," "foam on water," "a thread in water." The soggy pulse can combine with related pulse conditions to form compound pulses, such as soggy slow, soggy rapid, etc.
According to the "Pinghu Pulse Study · Body Condition Poem," it states: "Soggy shape floats finely, needs light pressing, floating weakly on water, strength forbearing. After illness or childbirth, there is still medicine, if a healthy person sees this, it's rootless."
What does it mean?
The first part means if a person has a soggy pulse after illness or childbirth, it's a sign of exhaustion of qi and blood, so it is understandable that the vital energy is insufficient to cause a soggy pulse, and it can be restored through medical treatment.
The latter part means it seems like a healthy person, but if you feel a soggy pulse, it's not good, indicating the patient's body is relatively weak.
How to treat it?
Lu Xuan had gained some understanding of the soggy pulse through his photographic memory, but he was still clueless about how to treat it, mainly because the patient had no discomfort apart from the fever, increasing the difficulty in diagnosis.
Even though Lu Xuan had recently been diagnosing with seemingly divine insight, those were more frequently encountered situations. In complex cases like this, he's still at a loss.
