Chapter 632: Chapter 631: A Close Call
Disaster struck without warning, and Yan Feifan’s mind instantly went into overdrive.
"Full-speed transfusion through both IV lines! Open up a third line for backup."
The assistants and nurses Baining Hospital had assigned to Yan Feifan were highly skilled and competent.
They sprang into action the moment Yan Feifan gave the order.
They hung the prepared blood bags, connected them to the backup transfusion line they had previously placed in Wang Zenglin, and cranked the transfusion rate to maximum.
Yan Feifan immediately activated the suction tube on the robotic surgical arm.
With the WHIR of the suction pump, bright red blood was rapidly drawn out.
Meanwhile, a 3D fluoroscopic model of the bullet’s position was already running in Yan Feifan’s mind. He replayed a dynamic analysis over and over, simulating the immediate aftermath of the bullet’s removal.
Based on this analysis, the simulation, and the sheer volume of blood, Yan Feifan made his diagnosis.
’Two small arterial branches right next to the lumbar vertebra must have been torn.’
Considering the bullet was already out, Wang Zenglin’s condition, and the location of the arteries, Yan Feifan formulated an emergency plan in seconds.
"Get me a laryngoscopy set, now!"
The circulating nurse nearby froze for a second when she heard the order. Just as she was about to ask for confirmation, she was met with Yan Feifan’s gaze, sharp as an arrow.
She didn’t dare question him again and bolted out of the operating room.
A laryngoscope is a piece of surgical equipment developed specifically for laryngeal procedures.
It provides an access channel that holds the mouth and throat open, allowing specialized, long-handled instruments to pass through. Surgeons can then perform delicate laryngeal operations using the view from the laryngoscope’s camera.
The procedure is actually quite similar to a minimally invasive laparoscopic surgery.
As the large volume of blood was suctioned away, the view from the camera on the surgical arm cleared up.
The lens on this camera had a special coating, making it highly resistant to water, oil, and adhesion.
’You get what you pay for. This lens wasn’t cheap.’
Through the restored laparoscopic view, Yan Feifan quickly located the two bleeding points, confirming his analysis.
This allowed him to relax slightly.
’It’s not a major artery. At this rate of bleeding, and with rapid transfusions, Wang Zenglin can hold on for a while longer.’
While the circulating nurse was still gone, Yan Feifan changed into a fresh pair of surgical gloves. He picked up a large, rounded scalpel and positioned himself behind Wang Zenglin, who was lying on his side.
Yan Feifan decisively made a seven-centimeter incision at the junction of Wang Zenglin’s lumbar spine and eleventh rib, creating a surgical access point.
As soon as the incision was made, pooled blood gushed out in a SPLASH, splattering all over Yan Feifan.
Unfazed, Yan Feifan took the retractor from the scrub nurse and used it to widen and secure the incision.
Next, holding a small hemostatic clamp between his index and middle fingers, he guided it into the incision.
Guided by the camera feed from the surgical arm, Yan Feifan located one of the bleeders. With deft and precise movements, his fingers manipulated the clamp to secure the upper end of the torn arterial branch.
Then, he repeated the process, clamping off the second arterial branch to stop the bleeding.
In the temporary viewing room at the information center, Yu Youlian couldn’t help but let out a long sigh of relief as she watched the scene unfold.
’The bleeding is stopped. The patient can hold on much longer now, giving them ample time for the next steps of the procedure...’
Just then, the voice of the man with the pencil mustache sounded in her ear.
"Sis, this Mr. Yan is really something else."
"He was faced with a sudden complication, but he didn’t show the slightest panic. His response was swift and effective, more seasoned than a surgeon with twenty or thirty years of experience."
Yu Youlian replied with a proud smile, "It’s his ability to perform under pressure, to never drop the ball at a critical moment, that earns him the title of medical genius."
After a brief pause, she added worriedly, "But you’re the one who’s better at laparoscopic vascular suturing."
"Youhai, why don’t you go get ready, just in case?"
The man with the pencil mustache shook his head. "Sis, I can’t just barge in unless the lead surgeon asks for help. It’s a professional taboo."
"Not unless I was his attending physician or superior."
"Look at how easily he’s handling this. Sis, I’m sure he knows what he’s doing."
The man with the pencil mustache added gravely, "A good, responsible doctor wouldn’t risk a patient’s life just to save face by overestimating their own abilities."
"Especially not on a live broadcast."
"Hey, Sis, how many people are watching, anyway?"
Yu Youlian picked up a tablet, tapped the screen a few times, and pulled up the admin page for the live stream.
"Holy crap, over six thousand people! And the number is still climbing fast..."
Another six minutes passed before the circulating nurse and two assistants finally rushed back, panting, with a complete laryngoscopy set.
Yan Feifan inserted the laryngoscope into the incision he had made between Wang Zenglin’s lumbar vertebra and rib. After carefully adjusting it, he used the view from both the laryngoscope and the robotic arm’s camera. He then inserted long-handled surgical forceps and began to suture the severed arterial branches.
Suturing a blood vessel—especially one as thick as 1.5 millimeters—was a piece of cake for Yan Feifan, even using the difficult-to-handle, long-shafted instruments.
The rest of the suturing was completed smoothly and skillfully by Yan Feifan. There were no further complications.
During the final phase of the operation, Yan Feifan personally sutured the two incisions—the one near Wang Zenglin’s lumbar spine and ribs, and the other on his left waist.
Once that was done, he glanced up at the monitor screens.
All his vital signs—heart rate, blood pressure, respiration, and blood oxygen saturation—were within acceptable ranges.
Yan Feifan glanced at the clock on the wall. The entire surgery had taken two hours and seventeen minutes.
"The rest is up to you."
Yan Feifan said to the assistants and nurses, then picked up the small resin box containing the cleaned, badly deformed bullet and left the operating room.
He stripped off his blood-stained surgical gown, tossed it into a disposal hamper, and walked out of the sterile zone in his blue short-sleeved scrubs.
WHOOSH. A crowd of people immediately surrounded him—Wang Zenglin’s family, friends, as well as his supervisors and colleagues from work.
Facing their concerned gazes, Yan Feifan said, "We had a complication during the surgery. It was a tense moment, but he’s not in any danger."
"Overall, the surgery was a success."
Yan Feifan then handed the small box containing the bullet to Wang Zenglin’s wife.
"This is the bullet that was lodged in his lumbar spine. You can keep it as a souvenir."
Wang Zenglin’s wife accepted the box, crying tears of joy. "Thank you, Mr. Yan, thank you!"
Mr. Yi also boomed, "Mr. Yan, thank you for your hard work. On behalf of the entire district bureau, I want to express our gratitude!"
Just then, someone started to applaud.
Seeing that the applause was about to spread through the crowd, Yan Feifan quickly raised his voice. "This is a critical surgical area. Please be quiet."
After halting the applause, Yan Feifan caught someone waving at him out of the corner of his eye.
He turned to look.
He saw his sister, and he saw Zeng Ni waving, along with Xu Yanhai, Sun Xuenong, Shang Xiaorong, and a pouting Zhao Xiaoting.
Next to them stood Yu Youhai, Yu Youlian, Yu Nanxing, and their group.
Yan Feifan gave them a quick wave in greeting, then turned and headed back into the surgical area to change.
